Who we are
ADHD Assessments Hull, Innovations in Care, 20–22 Chamberlain Road, Hull HU8 8HP, is the data controller for the personal information you give us. Contact: referrals@libertyassessments.co.uk.
What we collect
- Contact details: name, email address, phone number and date of birth.
- Booking details: the service, format and appointment time you choose.
- If you use the free self-check and agree to us contacting you: your contact details, your consent and your result.
- If you book group coaching: your name, email, phone number and any notes, which we keep securely with our website host (Netlify) to manage places on the course.
- Health information you share with us, including questionnaires, QbCheck results, clinical notes and reports (assessments), and coaching notes (coaching).
- Payment confirmation from Stripe. We do not see or store your full card details.
Why we use it, and our legal basis
We use your information to arrange and provide the service you've booked, to communicate with you, and to meet our legal and professional obligations. Our lawful basis is performance of a contract and, for health information, the provision of health care (UK GDPR Article 9(2)(h)).
After your assessment or coaching course, we may send you one email asking whether you'd like to leave a review. It's optional, we only ask once, and it contains no advertising. We do this in our legitimate interest in hearing about your experience. If you'd rather not get it, just tell us.
Who we share it with
- Stripe: to process payments.
- Microsoft 365: our calendar and email provider, to schedule your appointment.
- Netlify: our website host, which stores group coaching bookings.
- Qbtech: to deliver your QbCheck test (assessments only).
- Google: Google Analytics and Google Ads, only if you accept cookies (see below).
- Jotform: to securely receive your online questionnaire answers, and those of the person you ask to complete the childhood questionnaire (assessments only).
- Your GP: only with your permission.
We may share information without consent only where the law requires it, or where there is a serious risk of harm to you or others.
How long we keep it
Clinical records are kept in line with NHS records retention guidance. Other booking information is kept for as long as needed for accounting and legal purposes.
Your rights
You can ask to access, correct or delete your information, or object to how we use it. Email us to make a request. If you're unhappy, you can complain to the Information Commissioner's Office (ICO).
The free ADHD self-check
To start the self-check you enter your name, email and phone number. If you tick the optional box agreeing that we can contact you about your result, your details and your result are sent to us, and we keep a record of when you agreed. We use them only to talk to you about your result and whether a full assessment could help. You can withdraw this at any time by emailing referrals@libertyassessments.co.uk or replying to any message from us.
If you don't tick the box, your details and answers stay in your browser and are not sent to us.
Cookies
We only use analytics and advertising cookies if you choose Accept on our cookie message. If you choose Reject, or make no choice, they are not set.
- Google Analytics (cookies starting
_ga): shows us how many people visit, which pages they read and how they found us. Kept for up to 2 years. - Google Ads (cookies such as
_gcl_au): tells us whether our adverts led to a visit or booking. We don't use these for personalised or remarketing adverts.
If you've accepted cookies, we also tell Google when a booking, enquiry or call request is made, and the amount paid, so we can see which adverts work. We don't tell Google what you booked, your name or any health information.
We send Google the address of the page you're on, but never the private links in booking, questionnaire or coaching emails, and the Google tag isn't used on your questionnaire or coaching pages at all.
Some information is saved only in your own browser so the site works the way you set it: your reading comfort choices, your cookie choice, and your questionnaire progress. These aren't sent to us or anyone else.
You can change your mind at any time using Cookie settings (also in the footer of every page).
Booking terms and cancellation policy
Our booking terms, including cancellation, rescheduling and refunds, are in our Terms and Conditions.
