Terms and Conditions
For ADHD assessment and ADHD coaching services. Effective date: 1 October 2026.
These Terms and Conditions apply to services provided by Liberty Assessments Ltd (Company No. 16675493), trading as ADHD Assessments Hull (“we”, “us”, “our”).
Registered/business address: 20–22 Chamberlain Road, Hull, HU8 8HP
Email: referrals@libertyassessments.co.uk
Telephone: 01482 770330
Please read these Terms and Conditions before booking. By making a booking and accepting these Terms and Conditions, you enter into a contract with Liberty Assessments Ltd.
Nothing in these Terms and Conditions limits or excludes any rights you have under applicable consumer law.
1. Our services
We provide private services to adults aged 18 and over, including:
- adult ADHD assessment and diagnosis;
- individual ADHD coaching;
- group ADHD coaching;
- our 12-month ADHD coaching programme; and
- related reports, recommendations and additional services where agreed.
The particular service purchased, price and appointment details will be shown during the booking process or otherwise confirmed to you in writing.
We may decline to provide a service where we reasonably consider that the service is unsuitable, unsafe, outside our professional competence or scope, or where another service would better meet the person's needs.
Part A – General booking terms
2. Booking and payment
Payment is required in full in advance unless we expressly agree otherwise.
ADHD assessment is currently priced at £595.
Where available, customers may use third-party payment services such as Klarna or Clearpay. Any credit or instalment agreement entered into with a third-party payment provider is subject to that provider's separate terms and conditions.
A booking is not confirmed until the required payment has been received or an approved payment arrangement has been successfully established and we have confirmed the booking.
Prices will be displayed before purchase. We may change our prices from time to time, but this will not normally affect a service that has already been booked and paid for.
3. Your responsibility to provide accurate information
You must provide information that is complete and accurate to the best of your knowledge.
For ADHD assessment in particular, the quality and completeness of the information available can affect the clinician's ability to reach a reliable diagnostic conclusion.
You must not deliberately provide false or misleading information or deliberately withhold information that could reasonably be considered material to the assessment, your safety, another person's safety, or the clinician's professional decision-making.
If relevant information becomes available after booking, you should tell us or your clinician.
We recognise that people may genuinely forget information, be uncertain about historical events, have incomplete records or find some questions difficult to answer. This clause is not intended to penalise genuine mistakes, differences in recollection or information that was not reasonably known to you.
4. Cancellation by you
You should notify us as soon as possible if you cannot attend.
More than 24 hours' notice
Where you cancel more than 24 hours before the scheduled appointment, any refund will be subject to any work already undertaken and reasonable costs already incurred in providing your service.
Where no substantive work has commenced, we will normally refund the amount paid, less any applicable and reasonable administration costs.
Less than 24 hours' notice
If you cancel less than 24 hours before your appointment, a cancellation charge of up to 100% of the price of the affected appointment may apply.
Our normal online booking window closes 24 hours before an appointment. An appointment cancelled after this point will therefore ordinarily be unable to be offered to another customer through our normal booking process.
Any amount retained or charged is intended to reflect the loss reasonably and directly incurred because of the cancellation and is not intended to operate as a penalty.
Where we are able to avoid or reduce our loss, including where the appointment is subsequently filled, this will be taken into account when determining the amount retained or charged.
These provisions do not affect your statutory cancellation or consumer rights.
UK consumer guidance provides that cancellation charges should reflect the business's actual direct loss and reasonable steps to mitigate that loss. GOV.UK (opens in a new tab)
5. Rescheduling
You may request to reschedule an appointment once, subject to sufficient notice and a £25 administration fee.
A request to reschedule made less than 24 hours before the appointment may be treated as a late cancellation because the original appointment may no longer be capable of being reallocated.
Further requests to reschedule will be considered at our discretion and may incur additional charges.
We may waive a fee where we consider there are exceptional circumstances. Any decision to waive a fee on one occasion does not require us to do so on another occasion.
6. Late arrival and non-attendance
You should attend or join your appointment at the agreed time.
If you arrive late but within 30 minutes of the scheduled start time, the appointment may continue for the remainder of the originally booked period.
For example, if a 60-minute appointment begins 30 minutes late because of your late arrival, approximately 30 minutes of the original appointment will remain. The appointment will not normally be extended because of late arrival.
Where the reduced time means that an ADHD assessment cannot safely or professionally be completed, a further appointment may be required and an additional fee may apply.
If you are more than 30 minutes late, you will normally be recorded as having not attended.
A non-attendance may result in a charge of up to 100% of the affected appointment fee where this reflects the loss we have reasonably incurred.
7. Cancellation or rescheduling by us
Occasionally we may need to change an appointment because of clinician illness, emergency, technical problems or other circumstances outside our reasonable control.
Where this happens, we will offer an alternative appointment at no additional charge and make reasonable efforts to arrange it at a mutually convenient time.
If we are ultimately unable to provide a service for which you have paid, your statutory rights and remedies will continue to apply.
We are not responsible for consequential losses that could not reasonably have been anticipated when the contract was made, subject always to rights and liabilities which cannot legally be excluded.
8. Statutory cancellation rights for online bookings
Where applicable, consumers purchasing services online or at a distance have statutory cancellation rights, which will apply regardless of anything elsewhere in these Terms and Conditions.
Where you expressly request that we begin providing a service during a statutory cancellation period, you may be required to pay a proportionate amount for services already supplied if you subsequently exercise a statutory right to cancel.
Where a service has been fully performed during the cancellation period following the legally required request and acknowledgement, the statutory right to cancel may cease in accordance with applicable law.
Nothing in our cancellation policy removes or restricts these statutory rights.
For the 12-month coaching programme, the cooling-off period and cancellation terms are set out in clause 24A.
Part B – ADHD assessment
9. Nature of the ADHD assessment
Our ADHD assessment is a professional clinical assessment undertaken by an appropriately qualified healthcare professional with relevant experience and/or qualifications in areas including mental health assessment and ADHD assessment and diagnosis.
Our clinicians may come from different regulated healthcare professional backgrounds.
Assessments are undertaken with reference to appropriate professional standards and guidance, including relevant NICE guidance and DSM-5 diagnostic criteria.
An ADHD assessment involves clinical judgement and is not determined solely by questionnaire scores.
10. What the £595 ADHD assessment includes
The assessment package ordinarily includes:
- relevant pre-assessment questionnaires;
- clinical ADHD assessment;
- developmental and childhood history;
- consideration of current symptoms and functional impairment;
- consideration of information provided by an informant where available;
- consideration of relevant supporting documentation where available;
- consideration of relevant mental-health and differential-diagnostic factors;
- a professional diagnostic decision;
- a written assessment report;
- feedback regarding the assessment outcome;
- recommendations where appropriate; and
- a GP letter where clinically appropriate.
The precise assessment process may be adapted according to individual clinical circumstances.
11. A diagnosis is not guaranteed
Payment is for the assessment process and professional clinical opinion. Payment does not guarantee a diagnosis of ADHD.
Following assessment, the clinician may conclude that:
- the diagnostic criteria for ADHD are met;
- the diagnostic criteria for ADHD are not met;
- there is currently insufficient evidence to reach a reliable conclusion;
- further information is required;
- further assessment is required; or
- another condition or explanation should be investigated.
A decision not to diagnose ADHD does not entitle the customer to a refund merely because the outcome was not the one they expected or hoped for.
12. Questionnaires
Customers will ordinarily be asked to complete appropriate questionnaires as part of the assessment process. These may include measures relating to ADHD symptoms, childhood symptoms, functional impairment, mental health, autism screening and collateral/informant information.
Where questionnaires have not been completed before the appointment, they may be completed or explored during the assessment.
This may reduce the amount of appointment time available for other elements of the assessment and, where necessary, may result in further assessment being required.
Questionnaire results support clinical assessment but do not, by themselves, establish or exclude an ADHD diagnosis.
13. Childhood and informant information
ADHD assessment ordinarily requires consideration of developmental history and evidence relating to symptoms during childhood.
Information from someone who knew you during childhood is strongly preferred where reasonably available, but we recognise that this is not possible for every adult.
We may also consider school reports, medical records or other historical information where available.
The absence of an informant or school records does not automatically prevent an assessment or diagnosis. The clinician will determine whether sufficient reliable evidence is available to reach a professional diagnostic conclusion.
14. Further information or assessment
A clinician may defer a diagnostic decision where additional information is reasonably required.
This could include:
- further developmental information;
- additional questionnaires;
- information from an informant;
- relevant records;
- further clinical assessment;
- physical-health investigations;
- information from another healthcare professional; or
- assessment by another specialist.
Where additional appointments or services fall outside the assessment package originally purchased, we will tell you about any additional charge before that additional service is provided.
15. Other conditions and differential diagnosis
Symptoms associated with ADHD can overlap with other mental-health, neurodevelopmental, physical-health and other conditions.
Our assessment therefore considers reasonable alternative explanations and relevant co-occurring difficulties within the competence and scope of the assessing clinician.
An ADHD assessment is not automatically a comprehensive diagnostic assessment for every other possible condition.
Where another condition is suspected, we may recommend that you speak with your GP or seek assessment from an appropriate healthcare professional or specialist.
16. Assessment reports
We aim to provide completed ADHD assessment reports within five working days, Monday to Friday, following completion of the assessment and receipt of all information reasonably required to reach the clinical conclusion.
This is a target rather than an absolute guarantee.
Complex cases, outstanding collateral information, clinical review, quality assurance, staff absence or other reasonable circumstances may result in a longer turnaround time.
We will endeavour to notify you where there is a significant delay.
17. Corrections to reports
If you identify a genuine factual error in your report, such as an incorrect date, name or inaccurately recorded factual information, you may ask us to review it.
Genuine factual errors will be corrected where appropriate without charge.
You may provide additional information or explain where you disagree with a clinical conclusion, but a customer cannot require a clinician to alter an independent professional opinion, diagnostic conclusion or recommendation merely because they disagree with it.
The clinician retains professional responsibility for the contents of their report.
18. Additional and bespoke letters
The standard assessment report is included in the assessment price.
Bespoke reports or letters for purposes such as benefits applications, PIP, employers, universities, education providers, solicitors, insurers or other third parties are not necessarily included.
Where we agree to provide such documentation, an additional charge may apply. The price will depend upon the complexity and work required and will be confirmed before the work is undertaken.
19. Medication and prescribing
ADHD Assessments Hull does not currently provide ADHD medication prescribing or titration as part of the £595 assessment service.
Where medication may be appropriate, we may provide information about or refer/signpost you to an appropriate prescribing service.
The decision whether medication is appropriate remains a matter for the appropriately qualified prescriber.
ADHD coaching is not a substitute for prescribed medication or medical care.
20. NHS services and shared care
We cannot guarantee that an NHS GP, NHS specialist, private clinician or other healthcare provider will:
- accept a private diagnostic report;
- prescribe ADHD medication;
- accept responsibility for ongoing prescribing;
- enter into a shared-care arrangement;
- accept a referral;
- provide treatment; or
- agree with our clinician's opinion.
Those decisions are made independently by the relevant healthcare professional or organisation.
21. Use of reports by third parties
Our reports provide a professional clinical opinion based upon the information available at the time of assessment.
We cannot guarantee that an assessment or report will result in:
- Personal Independence Payment or another benefit;
- Disabled Students' Allowance;
- university or educational adjustments;
- workplace reasonable adjustments;
- Access to Work support;
- insurance acceptance;
- medication or NHS treatment;
- shared care; or
- any particular decision by another organisation.
The relevant third party remains responsible for its own decisions and eligibility criteria.
Part C – ADHD coaching
22. Nature of ADHD coaching
ADHD coaching is a collaborative, practical and goal-focused service intended to help individuals better understand themselves and develop approaches that work in their everyday lives.
Depending upon individual needs, coaching may explore areas including:
- organisation and planning;
- task initiation and follow-through;
- routines and habits;
- executive functioning;
- emotional awareness and self-regulation;
- overwhelm;
- communication;
- confidence;
- strengths;
- work and study;
- relationships;
- personal goals; and
- practical strategies for everyday life.
Coaching is personalised and no particular outcome is guaranteed.
23. Coaching is not clinical treatment
ADHD coaching is not a substitute for medical care.
Unless explicitly contracted as a separate appropriately regulated clinical service, coaching does not constitute:
- psychotherapy;
- counselling;
- psychiatric treatment;
- medical treatment;
- medication management; or
- an emergency or crisis mental-health service.
Where a coach considers that your needs fall outside the scope of coaching, they may recommend that you seek support from your GP or another appropriate professional.
You do not need a formal ADHD diagnosis to access our ADHD coaching service.
24. Individual coaching programme
Our six-week individual ADHD coaching programme is currently priced at £450.
Individual coaching sessions are normally 60 minutes.
The six-session programme must normally be completed within three months of commencement.
Unused sessions are not transferable to another person.
Unused sessions do not automatically entitle the customer to a refund, although this does not remove any statutory rights or any refund which may otherwise be due under consumer law.
Following completion of the initial programme, additional individual sessions are currently available at £75 per session.
24A. 12-month coaching programme
What it is and who can buy it
Our 12-month ADHD coaching programme is currently priced at £825, paid in full in advance. Where available, you may pay using Klarna or Clearpay, subject to clause 2.
The programme is only available to customers who have completed our six-week individual coaching programme. The programme lasts 12 months from the date of your first programme session. The sessions and support included will be explained to you before you buy and confirmed in writing afterwards.
Programme sessions are subject to clauses 4 to 7. A session cancelled with less than 24 hours' notice, or not attended, will normally count as a session received.
Your 14-day cooling-off period
You have the right to cancel the programme within 14 days of the day you buy it, without giving any reason ("the cooling-off period").
Your first programme session will not take place during the cooling-off period unless you expressly ask for it to start sooner when you buy. If you cancel within the cooling-off period:
- if no programme sessions have taken place, we will refund the full amount you paid; or
- if you asked for the programme to start during the cooling-off period and one or more sessions have taken place, we will refund the amount you paid less a proportionate amount for the sessions you have already received, charged at our standard individual session price of £75 per session.
To cancel, tell us clearly that you wish to cancel, by email to referrals@libertyassessments.co.uk, by phone on 01482 770330, or by writing to us at the address at the end of these Terms. You may use the model cancellation form below, but you do not have to. To meet the deadline, you only need to send your message before the cooling-off period ends.
We will make any refund without undue delay, and in any event within 14 days of the day you tell us you wish to cancel, using the same payment method you used (or, if you paid using Klarna or Clearpay, through that provider).
Model cancellation form
To: Liberty Assessments Ltd (trading as ADHD Assessments Hull), 20–22 Chamberlain Road, Hull, HU8 8HP, referrals@libertyassessments.co.uk
I hereby give notice that I cancel my contract for the following service: 12-month ADHD coaching programme.
Ordered on: [date]
Name: [your name]
Address: [your address]
Signature (only if this form is sent on paper):
Date: [date]
If you cancel after the cooling-off period
After the cooling-off period, you may still end the programme at any time by telling us in the same way. The programme fee reflects a significant saving compared with booking sessions individually, and we reserve time in our diary for you across the year. So if you cancel after the cooling-off period, we will refund the amount you paid less:
- £75 for each programme session you have already received (our standard individual session price, rather than the discounted programme rate); and
- an administration fee of £25.
If these amounts equal or exceed the amount you paid, no refund will be due. For example, if you cancel after 4 sessions, we will refund £825 − (4 × £75) − £25 = £500. If you cancel after 11 or more sessions, no refund will be due.
We consider these deductions to be a fair reflection of the value of the service you have received and our reasonable costs. They are not intended to operate as a penalty and do not affect your statutory rights.
If you simply stop attending without telling us, the programme will continue until the end of its 12 months, and you can still cancel under this clause at any time before then. Sessions not used by the end of the 12 months will expire.
If we end the programme
If we end the programme early for a reason other than your serious breach of these Terms (see clause 34), or are unable to continue providing it, we will refund the proportion of the amount you paid that relates to the part of the 12 months not yet provided, calculated pro rata by time.
25. Group coaching
Our group ADHD coaching programme is currently priced at £295.
Group coaching necessarily involves interaction with other participants.
All participants are expected to respect the privacy and confidentiality of other group members and must not disclose another participant's personal information or experiences outside the group without that person's permission.
However, because other participants are independent individuals, we cannot guarantee that another participant will maintain confidentiality.
You should therefore only share information in a group session that you feel comfortable sharing with the other participants.
We may remove a participant from a group where their behaviour seriously compromises the safety, confidentiality or wellbeing of other participants.
26. Coaching engagement and outcomes
Coaching is collaborative. Its effectiveness can be influenced by attendance, engagement, personal circumstances and opportunities to apply strategies between sessions.
We will provide the service with reasonable care and skill, but we do not guarantee a particular personal, educational, employment, financial, health or other outcome.
UK consumer law requires services to be performed with reasonable care and skill. Legislation.gov.uk (opens in a new tab)
27. International coaching
We may provide coaching to customers outside the United Kingdom, including customers located in the United States.
International availability is subject to us first being satisfied that the service can lawfully and appropriately be provided in the customer's jurisdiction and remains within the professional scope of the person delivering it.
We reserve the right to decline or discontinue international coaching where legal, regulatory, insurance, professional or safeguarding considerations mean that we cannot appropriately provide the service.
Nothing in these Terms removes mandatory consumer rights which apply to a customer under the law of their jurisdiction where those rights cannot lawfully be excluded by choosing English law.
Part D – Confidentiality, safety and information
28. Confidentiality
Information disclosed during assessment or coaching will normally be treated confidentially and handled in accordance with applicable law, professional obligations and our Privacy Notice.
Confidentiality is not absolute.
Information may be disclosed without your permission where disclosure is reasonably necessary or legally justified, including circumstances involving:
- serious concerns about your safety;
- serious concerns about another person's safety;
- safeguarding concerns involving a child or adult at risk;
- prevention or detection of serious crime where applicable;
- a legal requirement or court order; or
- another lawful professional or regulatory obligation.
Where appropriate and safe to do so, we will normally seek to discuss such disclosure with you.
29. GP information
ADHD assessment customers are required to provide details of their GP or usual primary healthcare provider where applicable.
Your completed assessment report will ordinarily be provided to you.
It is normally your responsibility to provide the report to your GP or another professional unless another information-sharing arrangement has been agreed.
Where a significant clinical or safeguarding concern arises, we may contact your GP, emergency services, safeguarding services or another appropriate service where doing so is legally and professionally justified.
30. Emergency and crisis situations
Neither ADHD assessment nor ADHD coaching provided by us is an emergency or crisis service.
Emails, telephone messages, booking systems and coaching communications must not be relied upon for urgent or emergency assistance.
If you believe that you or another person is in immediate danger or experiencing a medical emergency, you should contact the appropriate emergency service.
For urgent health or mental-health concerns, you should use an appropriate NHS or emergency service according to the urgency of the situation.
31. Recording appointments
You must not audio-record, video-record, photograph, livestream or otherwise make a recording of an assessment, coaching session or group session without prior agreement.
We may occasionally wish to record or transcribe an appointment for legitimate purposes such as assisting with accurate report preparation, professional supervision, training or quality assurance.
Where we propose to make such a recording, you will be informed appropriately and the applicable consent, confidentiality, data-protection and retention requirements will apply.
Any recording will be handled as confidential information in accordance with our Privacy Notice and applicable data-protection requirements.
Health and diagnostic information constitutes special-category personal data and is subject to additional protections under UK data-protection law. ICO (opens in a new tab)
32. Privacy and personal information
We process personal information, including health information, in connection with providing our services.
Our Privacy Notice explains:
- what information we collect;
- why we collect it;
- our lawful bases and applicable special-category conditions;
- who information may be shared with;
- how long information is retained;
- your data-protection rights; and
- how to contact us about your information.
Our Privacy Notice is available through the ADHD Assessments Hull website.
Acceptance of these contractual Terms and Conditions does not itself constitute consent to every possible use of personal information. Data will be processed according to the applicable lawful basis and requirements of UK data-protection law.
Part E – Professional and behavioural standards
33. Respectful behaviour
We expect customers, clinicians, coaches and staff to treat one another with dignity and respect.
We will not tolerate serious or persistent:
- threatening behaviour;
- violence or threats of violence;
- abusive or intimidating behaviour;
- harassment;
- discriminatory abuse;
- sexual harassment or inappropriate sexual behaviour; or
- conduct that creates an unacceptable risk to staff, clinicians, other customers or group participants.
Where necessary, an appointment may be immediately terminated.
34. Our right to decline or discontinue a service
We may decline, suspend or discontinue a service where there is a reasonable professional, clinical, safety or contractual reason for doing so.
Examples include:
- serious aggression, threats or abusive behaviour;
- deliberate provision of materially false information;
- deliberate withholding of information material to safe or appropriate assessment;
- fraud;
- serious or repeated breach of these Terms;
- a conflict of interest;
- the service being clinically inappropriate;
- the customer's needs falling outside our professional competence or service scope;
- continuing the service creating an unacceptable safety risk; or
- circumstances in which professional standards require the clinician to withdraw.
Where reasonably possible, we will explain the decision.
If we discontinue a service, the financial position will depend upon the circumstances, including the work already undertaken and losses reasonably incurred.
We will not automatically retain payment for services that have not been provided merely because we have exercised our right to terminate.
However, where termination results directly from the customer's serious breach or conduct and we have already incurred costs or suffered direct loss, we may retain or recover a reasonable amount reflecting that work or loss, subject to applicable consumer law.
Part F – Complaints
35. Complaints
We welcome the opportunity to resolve concerns about our services.
Complaints should initially be addressed to:
Daniel Fellows
Managing Director
Liberty Assessments Ltd
Email: referrals@libertyassessments.co.uk
Please provide sufficient information for us to understand and investigate the concern.
We will acknowledge and investigate complaints fairly and will provide an outcome following appropriate review.
Making a complaint will not result in adverse treatment or affect the impartiality of any clinical decision.
Where a complaint relates to the professional conduct of an independently regulated healthcare professional, you may also have the right to raise concerns with that professional's regulatory body.
Part G – Liability and consumer rights
36. Standard of service
We will provide our services with reasonable care and skill and in accordance with applicable professional and legal requirements.
Nothing in these Terms excludes or restricts liability where doing so would be unlawful.
In particular, nothing in these Terms limits your statutory rights under the Consumer Rights Act 2015 or other applicable consumer legislation. The Consumer Rights Act implies a term into consumer service contracts requiring reasonable care and skill. Legislation.gov.uk (opens in a new tab)
37. Matters outside our control
We are not responsible for decisions made by independent third parties, including GPs, NHS organisations, employers, universities, benefits authorities, insurers, prescribers or other healthcare providers.
We are also not responsible for losses caused solely by inaccurate or materially incomplete information supplied to us where we reasonably relied upon that information.
Nothing in this section excludes liability which cannot lawfully be excluded.
38. Online appointments and technology
For online appointments, you are responsible for having a reasonably suitable internet connection, compatible device and private environment where appropriate.
You should join the appointment in sufficient time to check that your equipment is working.
Where a significant technical problem attributable to us prevents the appointment from proceeding, we will normally rearrange the affected appointment without additional charge.
Where technical problems on the customer's side prevent attendance or substantially disrupt an appointment, we will consider the circumstances reasonably when determining whether the appointment should be rearranged or a fee applied.
39. Intellectual property
Reports supplied to you may be used for your own personal purposes and may be provided to relevant professionals or organisations for legitimate purposes connected with your care, support, education, employment or other needs.
Copyright and other intellectual-property rights in our original questionnaires, coaching resources, worksheets, course materials, training materials and other proprietary resources remain with us or the relevant rights holder.
You must not reproduce, sell, publish, commercially distribute or use such materials to provide services to others without permission.
40. Changes to these Terms
We may update these Terms and Conditions periodically to reflect changes in our services, professional requirements or applicable law.
The terms applicable to an existing booking will normally be those accepted when the contract was made, except where a change is required by law or is expressly agreed with you.
41. Severability
If any provision of these Terms is found to be unlawful or unenforceable, the remaining provisions will continue to apply so far as legally possible.
42. No waiver
If we choose not to enforce a provision on one occasion, this does not prevent us from enforcing it on another occasion.
43. Governing law
These Terms and Conditions are governed by the laws of England and Wales.
Customers ordinarily resident elsewhere retain any mandatory consumer protections that apply to them and which cannot lawfully be excluded by these Terms.
44. Acceptance
By booking a service and accepting these Terms and Conditions, you confirm that:
- you are aged 18 or over;
- you have had the opportunity to read these Terms and Conditions before purchasing;
- you understand the service you are purchasing;
- you agree to comply with these Terms and Conditions; and
- the information you provide will be accurate to the best of your knowledge.
For ADHD assessment, you also acknowledge that payment is for an independent professional assessment and clinical opinion and does not guarantee a diagnosis of ADHD.
These Terms and Conditions form part of the contract between you and Liberty Assessments Ltd.
